Health Information Technology and Quality Webinar: “Using Health IT for Care Coordination Across Inpatient and Outpatient Settings”

This webinar focuses on using health information technology (IT) to help coordinate care among outpatient and inpatient safety net providers.  Care coordination between these two provider settings is important for improving healthcare outcomes, increasing patient safety, reducing costs, and attaining patient centered care. Presenters, from both rural and urban safety net environments, will discuss how they have achieved patient care coordination and describe how they overcame barriers related to infrastructure/ health IT, governance issues, staff communication, patient engagement, and the sharing of patient data. This webinar is important for safety net providers as they implement health IT systems and prepare for changes in how they provide patient care for programs funded by the Affordable Care Act.  Presenters include:

National Diabetes Education Program: Promoting Health after Gestational Diabetes
Monday, May 14th, 2012

During this webinar, Robert Ratner, M.D., F.A.C.P, F.A.C.E., Chief Scientific and Medical Officer of the American Diabetes Association, will discuss the lifelong health risks for women with a history of gestational diabetes, the health risks to a child of a pregnancy complicated by gestational diabetes, and steps mothers can take to help lower these risks for themselves and their children.

In addition, NDEP partner Thomas Joyce will share the Ohio Diabetes Prevention and Control Program’s strategies for promoting type 2 diabetes prevention among women with a history of gestational diabetes and their children.

Continuity and Resiliency for Health IT Systems:Preparing for Unforeseen Events
Friday, April 27, 2012 – 2:00PM ET 

This webinar provides expert advice on how health information technology (IT) can help primary care and rural inpatient providers prepare for and overcome an unforeseen event. When these events – such as a flood, data loss, power outage, snowstorm, or a public health crisis like an influenza outbreak – occur, health IT systems can be a valuable tool to ensure continuity of patient care. This is just one reason why it is critical to ensure that health IT systems are resilient and that staff can continue providing care under such circumstances.

Speakers will present case studies and lessons learned based on their own experiences of how their health IT systems helped them prepare for and overcome these events. They will also focus on how safety net providers can build a resilient health IT infrastructure and system, as well as work with local and state government entities to prepare for an unforeseen event.

Expanding Health Center Enabling Services: The Health Leads 
Thursday, April 12, 2012 
3:00-4:30pm ET

Enabling services are non-clinical services that make it possible for patients to access preventive and primary health care services.  Enabling services, which may include case management, referrals, translation/interpretation, transportation and health education, can play a major role in health care access and continuity of care.  Health Leads is a national healthcare organization that places volunteers in clinics to help connect patients to the resources they need to be healthy. In the hospitals and health centers where Health Leads operates, doctors can "prescribe" food, housing, or other critical resources just as they would medication.

We invite you to join a multi-disciplinary panel of Federal staff and BPHC grantee partners for this technical assistance call on how to expand enabling services at your health center to improve health outcomes for your population.

BPHC TA Enrichment Call: Stepping Up to Healthy Weight: Reducing and Preventing Overweight and Obesity in Health Center Populations
February 28, 2012
2:00PM – 3:30PM ET

The purpose of this enrichment session is to provide an overview of the obesity epidemic in the US, identify the importance of achieving a healthy weight in health center populations, describe some of the challenges health center patient populations face in maintaining healthy weight, and to provide presentations and lessons learned from two current BPHC health center grantees who will discuss their approach to achieving healthy weight in patient populations.

BPHC Grantee TA Enrichment Call: Stopping a Silent Epidemic: Policy and Practice Innovations to Treat and Prevent Viral Hepatitis
January 26, 2012

This session includes a clinical review of viral hepatitis, an overview of the HHS Viral Hepatitis Action Plan, and presentations from three current BPHC health center grantees who will discuss their approach to the treatment of Hepatitis B and C. The session concludes with a presentation on Project ECHO, an innovative program that utilizes telehealth in the treatment of viral hepatitis.
