Health centers meeting or exceeding the 2017 Million Hearts goal of 70% on the following UDS clinical performance measures: Heart Attack/Stroke Treatment (Aspirin Therapy for Ischemic Vascular Disease Patients), Blood Pressure Control (Hypertensive Patients with Blood Pressure < 140/90), and Tobacco Cessation Counseling for Tobacco Users.
