Overview

The American Rescue Plan provides one-time funding (HQC) for a 2-year period of performance to Health Center Controlled Networks (HCCNs). Funding will be used to support participating health centers (PHCs) to leverage health information technology and data to respond to and mitigate the spread of COVID-19, and enhance health care services and infrastructure.

HRSA awarded approximately $10.5 million to current HCCNs. Each HCCN’s award amount is equivalent to 25 percent of its FY 2020 HCCN operational (H2Q) grant award. The American Rescue Plan award notifications include a grant award number beginning with HQC that is unique to this funding and separate from the H2Q award.

See the HQC Award Submission Guidance for information about allowable uses of funding and instructions on how to complete and submit the following elements of your HQC Award Submission by June 28, 2021.

Progress Reporting

As stated on your notice of award, you must submit HQC quarterly reports and a final report in EHBs. HRSA will notify you about the due dates for each report, and will share additional resources on what they will include.
