The Patient-Centered Medical Home (PCMH) Recognition Quality Improvement Awards (QIA) recognize health centers’ commitment and investment in quality improvement activities.

Health centers that have achieved PCMH recognition by July 1, 2020, are eligible to receive a 2020 Health Center PCMH QIA. The award amount will be determined based upon the availability of federal funding.

Please review the PCMH Verification table (PDF - 341 KB) , which lists HRSA-funded health centers with valid PCMH recognition status and the number of PCMH-recognized sites for each. If your health center serves multiple states, please be certain to check the number of sites for EACH state to calculate the total number of PCMH-recognized sites in HRSA’s Electronic Handbooks (EHBs).

  • If the information is correct, you do not need to take any further action.
  • If the information is incorrect, please submit an update to your PCMH recognition via EHBs using the instructions below. Corrections must be submitted by Sunday, July 5.

Submitting an Update to your PCMH Recognition via EHBs

If your PCMH information is incorrect, please submit a correction to us through EHBs.

  1. Log in to your EHBs Grant Folder.
  2. Click the “HRSA Accreditation/PCMH Initiative” link.
  3. Choose the “Request PCMH Recognition Update” button and follow the instructions.

You must provide documentation of PCMH recognition (e.g., official certificate, copy of the email confirmation from the recognition body, or a screenshot of your information on the recognition body’s website). We will review all updates and confirm our review within EHBs.
