FY 2018 Expanding Access to Quality Substance Use Disorder and Mental Health Services (SUD-MH) (HRSA-18-118) Minor Alteration/Renovation (A/R) Capital Development Resources

Award recipients requesting one-time funding of up to $175,000 to support minor alternation and renovation (A/R) activities are required to submit a prior approval in EHB requesting a capital update to modernize, improve, and/or reconfigure the arrangements or other physical characteristics of an existing facility. 

Use this link to access the SUD-MH Minor A/R Capital Update Instructions (PDF – 243 KB). The capital update submission should include the following required documentation: an alternation/renovation budget justification (PDF – 119 KB), schematic drawings, and a completed Environmental and Information Documentation (EID) checklist (PDF – 351 KB), and a completed Other Requirements for Sites document (PDF - 132 KB) (note: for leased properties, please include a Landlord Letter of Consent -Minor Alteration/Renovation (PDF - 70 KB)).

Post-Award Capital-Related Conditions and Reporting Requirements

After reviewing of the capital update, HRSA may determine the need for additional information and/or documentation in the form of an award condition in order to satisfy program guidance and requirements. Throughout the course of the project period, award recipients are required to submit specific reporting requirements directly related to minor A/R activities in EHB listed below: 

  • Project Implementation Certificate template (PDF – 189 KB) documenting engagement with an architecture/engineering firm; and construction and design in accordance with the pertinent laws, codes, and guidelines.
  • Project Completion Certificate template (PDF – 192 KB) attaching a certificate of substantial completion and certificate of occupancy.
  • Photographs depicting the project before, during, and after the minor A/R activities have been implemented.
