The Fiscal Year (FY) 2019 Health Center Controlled Networks (HCCN) funding opportunity will increase health center participation in value-based care. Through leveraging health information technology (health IT), HCCNs will support health centers to:

  • Enhance the patient and provider experience
  • Advance interoperability
  • Use data to enhance value

HRSA plans to award approximately $42 million to an estimated 50 HCCNs. This funding opportunity is authorized by Section 330(e)(1)(C) of the Public Health Service Act, as amended.

Who Can Apply

Applicants must be one of the following:

  • A network (referred to as an HCCN) that is at least majority controlled and, as applicable, at least majority owned by Health Center Program award recipients (i.e., the majority of the governing board members are Health Center Program award recipients); or
  • A current Health Center Program award recipient, funded for at least the 2 consecutive preceding years, applying on behalf of an HCCN.

How to Apply

HCCN applications require a two-phase submission process.

Phase 1 - must be completed and successfully submitted to by 11:59 pm ET on January 3, 2019.
