Health Center Patient Survey
Overview
The Health Center Patient Survey (HCPS), sponsored by HRSA, provides robust patient-level data to determine how well health centers funded under Section 330 of the Public Health Service Act provide access to primary and preventive health care. The survey explores:
- How well health centers meet the health care needs of the medically underserved; and
- How health center patients perceive the quality of their care.
The HCPS offers a snapshot of the following about health center patients through self-report:
- Sociodemographic characteristics;
- Health conditions;
- Health behaviors;
- Access to and utilization of health care services;
- Satisfaction with health care services.
HRSA conducts the HCPS every five years. Survey results come from in-person, one-on-one interviews with patients, and are nationally representative of the Health Center Program patient population.
With support from the U.S. Department of Health and Human Services’ Office of the Assistant Secretary for Planning and Evaluation (ASPE), survey data at the patient level are available to download.

2014 Data Files
There are 7,002 records for 2014. This is the first year data are available to the public. The data files are provided in SAS, SPSS, and Stata formats. Once you submit the data user agreement, you can download any or all formats.
