Project Work Plan Instructions
Upload a Project Work Plan into Attachment 1 that details the proposed activities and timeline of your AxCS project to help you achieve the two AxCS objectives:
- Increase the number and percentage of patients screened for at least one of the following cancer types: cervical, breast, or colorectal cancer.
- Increase the number of patients assisted with accessing appropriate follow-up care (e.g., diagnostic services, therapies, clinical trials) within 30 days of receiving an abnormal cancer screening test result.
You will also track the percentage of patients that you refer for care and treatment for whom you receive a report from the provider to whom the patient was referred.
The sample below shows how you might structure your project work plan to ensure that you address all 4 required focus areas (access and affordability, patient experience, screening, and workforce development). You may add rows to have a single row for each activity for ease of organization and readability. See section IV.2.v. of the FY 2022 AxCS notice of funding opportunity for guidance on completing your project work plan.
