Attachment 12: Operational Plan Instructions and Sample

New and competing supplement applicants must upload a detailed operational plan which addresses each of the following focus areas:

  1. Operational Service Delivery: Show that within 120 days of release of the Notice of Award (NOA) all sites will have the staff and providers you need to begin operating and delivering services. Make sure that sites and services are consistent with Form 5B: Service Sites (PDF - 161 KB) and Form 5A: Services Provided (PDF - 242 KB).
  2. Contracts and Subawards: Detail plans to hire, contract, and/or establish cooperative arrangements with all providers. Show that you will be able to provide all services at all sites for the stated number of hours within one year of the NoA release.
  3. Compliance: Demonstrate compliance with Health Center Program requirements for Quality Improvement and Quality Assurance, program monitoring, and data reporting.
  4. Board Requirements: Meet the Health Center Program requirements related to board authority, board composition, and conflicts of interest.
  5. Special Medically Underserved Population: If you apply for service areas with special medically underserved population funding, you must include an additional focus area to detail your plans for ensuring that required services are available to each population.

Table 1: Required Components of the Operational Plan

Provide all of the following details for each of the four focus areas of the Operational Plan.  If you will serve a special medically underserved population, you will have a fifth focus area and must address the additional the details outlined below.
