Requesting Temporary Service Sites in Response to Emergency Events: Resource and Submission Template

When to Use This Resource

Health centers use this optional resource to:

  1. Assess whether a proposed site is eligible for the streamlined request process to add a temporary service site in response to a declared emergency; and
  2. Submit the temporary service site request information to HRSA.

When NOT to Use This Resource

Not every health center location or activity may meet the temporary service site criteria described in PAL 2020-05 (PDF - 266 KB) and PIN 2008-01: Defining Scope of Project and Policy for Requesting Changes (PDF - 224 KB). Situations that may NOT meet the criteria for requesting a temporary service site could include, but are not limited to, the following:

  • The location(s) or activity(ies) may already be in the health center’s scope of project or may not rise to the level of a service site request.
    • As examples: Home visits that the health center already has recorded on its Form 5C: Other Activities/Locations or a location where services are already recorded on the health center’s Form 5A: Services Provided are exclusively delivered via telehealth.

  • A location that does not meet the criteria in PAL 2020-05 (PDF - 266 KB) for requesting a temporary service site may meet the definition of a permanent service site or permanent administrative site (refer to the Self-Assessment Checklist).
  • Activities may be conducted as other lines of business outside the health center’s scope of project.
    • A health center may decide to conduct activities outside the health center’s scope of project as other lines of business.
