FY 2025 PCA NCC Progress Report: Sample Staffing Plan

Instructions 

The fiscal year (FY) 2025 Primary Care Associations (PCAs) Non-Competing Continuation (NCC) progress report requires an updated staffing plan (submitted as Attachment 2: Staffing Plan) only if it has changed, or you anticipate changes prior to the end of the period of performance (June 30, 2026).

Use this sample staffing plan document for reference only. Refer to the FY 2025 PCA NCC progress report instructions available on the PCA TA webpage when creating your updated staffing plan. NOTE: Combined time and effort percentages of staff across all federal awards may not exceed 1.0 FTE.
