Who can apply

You can be:

• Currently funded or newly formed HCCNs
• A current Health Center Program (H80) award recipient that has been funded for at least two consecutive preceding years applying on behalf of an HCCN
• Public and private institutions of higher education
• Domestic public or private nonprofit entities
• Domestic faith-based and community-based organizations
• Native American tribes, and tribal organizations

To be an HCCN, you must meet the following criteria throughout the period of performance: 

• Have at least 10 participating health centers (PHCs) 
• Be majority controlled and, as applicable, at least majority owned by Health Center Program (H80) award recipients
• Have its own governing body independent of the boards of its health center members

We will award approximately $48 million to fund 49 organizations.

Our funding is for HCCNs to support PHCs as they leverage health IT and data to enhance delivery of affordable, accessible, and high-quality primary health care, with a specific focus on:

• Data management and analytics
• Interoperability of systems and digital health tools
• Uniform Data System Patient-Level Submission (UDS+) implementation

How to apply

Apply in both systems:

1. Grants.gov: Apply by 11:59 p.m. ET on December 2, 2024.
2. HRSA Electronic Handbooks (EHBs): by 5 p.m. ET on January 10, 2025.
