Who can apply
You can be:
- Currently funded or newly formed HCCN
- Current Health Center Program (H80) award recipient that we funded for at least two years before you apply on behalf of an HCCN
- Public or private school of higher education
- Domestic public or private nonprofit entity
- Domestic faith-based or community-based group
- Native American tribe or tribal group
To be an HCCN, you must meet the following criteria throughout the period of performance:
- At least 10 participating health centers (PHCs)
- Majority controlled and at least majority owned by Health Center Program (H80) award recipients
- Governing body independent of the boards of your health center members
Get key funding details
We will award roughly $48 million to fund 49 HCCNs to support PHCs.
You’ll need to leverage health IT and data to improve how you deliver primary health care. Care must be affordable, accessible, and high-quality. Focus on:
- Managing data and analytics
- Making systems and digital health tools work together
- Submitting Uniform Data System Patient-Level (UDS+) data
How to apply
Apply in both systems:
- Grants.gov: Apply by 11:59 p.m. ET on December 2, 2024.
- HRSA Electronic Handbooks (EHBs): Apply by 5 p.m. ET on January 10, 2025.
