Sample HCCN Communications Plan

The fiscal year (FY) 2023 Health Center Controlled Networks (HCCNs) Non-Competing Continuation (NCC) progress report requires an updated Communications Plan only if it has changed, or you anticipate changes to the Communications Plan submitted with your FY 2022 HCCN application. Use this sample for reference only. Refer to the FY 2023 HCCN NCC Progress Report Instructions available on the HCCN TA webpage for detailed instructions.

Identify and address PHCs’ needs and challenges

Solicit needs and challenges encountered by PHCs through optional web-based survey (scheduled to optimize responses and reduce survey fatigue) hosted on HCCN web-portal. Included phone-based follow-up. Results will be used to create targeted messaging about available health IT and data resources to subsets of PHCs based on identified needs, challenges, and interests.

Performance feedback
