Sample HCCN Summary of Contracts and Agreements
The fiscal year (FY) 2024 Health Center Controlled Networks (HCCNs) Non-Competing Continuation (NCC) progress report requires an updated Summary of Contracts and Agreements only if it has changed, or you anticipate changes before the end of the period of performance (July 31, 2025).
Use this sample for reference only. Refer to the FY 2024 HCCN NCC progress report instructions available on the HCCN TA webpage for detailed instructions.
NOTE: Your Summary of Contracts and Agreements must align with your budget narrative. You must exercise appropriate oversight and authority over all new contracts, and procurement contracts must comply with 45 CFR part 75.
