We provide this sample to show the level of detail we expect from you on your FY 2025 Primary Care Association Noncompeting Continuation (PCA NCC) Project Work Plan. 

The Plan outlines your activities for the FY 2025 budget period. These help you attain your objective targets.

Your FY 2025 PWP form will have information from the most recently approved PWP and the FY 2024 Progress Update form. 

The PCA NCC instructions explain how to complete the fields.

Objective Impact Narrative 

We expect to train 150 health centers on identifying and addressing disparities in care delivery settings to improve health outcomes by the end of the period of performance. The training will focus on the clinical quality measures used for health centers to earn the Health Disparities Reducer CHQR Badge: low birth weight, controlling high blood pressure, and uncontrolled diabetes. 

We expect that this training will contribute to an increase in the number of health centers that meet the clinical quality benchmarks and to be awarded the Health Disparities Reducer CHQR Badge.

Activity Description*

Due to the success of this activity and continued demand for this training in the first budget year, we will offer updated virtual trainings that incorporate feedback we received from when we held the training in September 2024. 
We will deliver virtual trainings to health centers on establishing health center leadership committees that guide efforts to improve health outcomes. Building upon our experience working with governance and oversight committees, one training will have a target audience of health center leadership and board members. At the same time, another will be a complementary session for clinicians and other health center staff.  

Using updated UDS data, we will promote the T/TA learning series to all health centers whose clinical quality outcomes for low birth weight, controlling high blood pressure, and uncontrolled diabetes did not meet the benchmarks to earn the Health Disparities Reducer CHQR Badge. 

We will continue to promote the recorded webinars to meet our goal of 100 downloads of the associated summary/one-pager. We will record the virtual trainings and post them and the related publications within 2 weeks, the same as last year.

We will provide the content for the training and publication. XYZ PCA will recruit health center clinicians and staff to participate in the virtual training and follow up with participants on applying the content discussed in the webinar during the peer groups it hosts.

Anticipated Outcome* At the conclusion of each of the two virtual trainings, leaders from at least 15 health centers will be able to identify the key functions and activities of a committee to guide health center efforts, as well as the process for forming and developing a committee. We anticipate that there will be at least another 100 downloads and/or views of the associated summary/one-pager by the end of the second budget year.
Comments In years 2 and 3, we will continue to grow our experience working with governance and oversight committees. By actively engaging health center leadership along with clinicians and staff, we expect that a strong organizational focus will develop and lead to improved clinical outcomes.
