Fiscal Year 2021 National Training and Technical Assistance Partners Non-Competing Continuation Progress Report: Sample Summary of Contracts and Agreements

Instructions for Completing the Summary of Contracts and Agreements 

The fiscal year (FY) 2021 National Training and Technical Assistance Partners (NTTAP) Non-Competing Continuation (NCC) Progress Report requires an updated Summary of Contracts and Agreements (submitted as Attachment 6) only if revisions have been, or will soon be, made. Use this sample Summary of Contracts and Agreements document for reference only.

If your contracts and agreements have changed since you submitted your FY 2020 NTTAP application, or are forecasted to change prior to June 30, 2021, upload a revised Summary of Contracts and Agreements. The summary must include the following elements:

  • Name of organization,
  • Type of contract or agreement (e.g., contract, Memorandum of Understanding or Agreement),
  • Brief description of the purpose and scope of the contract/agreement, including how and where services are, or will be, provided, and
  • Timeframe for the contract/agreement (e.g., ongoing contractual relationship, specific duration).

NOTE: Your Summary of Contracts and Agreements must align with your Budget Narrative. You must exercise appropriate oversight and authority over all new contracts, and procurement contracts must comply with 45 CFR part 75.
