The commitment

As a Health Resources and Services Administration (HRSA)-supported health center, we are committed to improving patient outcomes by advancing evidence-based, high-quality, patient-centered comprehensive primary care.

We commit to the extent possible to advance best practices that will:

Prioritize prevention and lifestyle

Help patients understand that many chronic diseases can be prevented, delayed, better managed, and reversed through healthier everyday choices and appropriate preventive care. Encourage lifestyle interventions—including a healthy diet, increased physical activity, adequate sleep, limited screen time, and other healthy behaviors—as first lines of intervention in building and maintaining health.

Promote healthy eating

Promote healthy eating and help patients and families understand how to apply the new Dietary Guidelines for Americans, 2025-2030 in everyday life. Emphasize the importance of whole, nutrient-dense foods including fresh fruits and vegetables; protein from both animal and plant sources; healthy fats, including full-fat dairy; and 100% whole grains. Educate patients and families about the significant harms associated with highly processed foods, added sugars, and refined grains and carbohydrates.

Empower every patient and family

Use the trusted relationship between patients and their health care teams to provide clear, practical, and actionable information about nutrition, physical activity, sleep, preventive screenings, limited alcohol consumption, and other healthy behaviors. Recognize that healthy habits begin at home and equip parents and caregivers with information they can use to improve the health of their children and families.

Connect patients to community health activities

As available, connect patients to community health activities such as community gardens, fresh produce prescriptions, teaching kitchens and food education classes, community exercise activities, and other programs that promote healthy lifestyle interventions.

Strengthen the prevention workforce

Equip physicians, nurses, community health workers, nutrition professionals, and other health center staff with the tools, training, and resources needed to make prevention, nutrition, and healthy living part of everyday patient engagement.

Build a culture of health

Promote an environment where healthy choices are encouraged, prevention is celebrated, and patients leave with greater knowledge, confidence, and practical tools to take control of their health.

This voluntary commitment is not a minimum standard of care, federal mandate, or replacement for clinical judgment. It is a shared commitment to advancing patient outcomes through evidence-based, high-quality, patient-centered care. A health center’s decision to sign or not sign the commitment will not be considered in connection with any pending or future application for HRSA funding and will not affect its eligibility for or receipt of HRSA funding. The activities described in this commitment do not establish requirements or preferences for HRSA funding opportunities.

Participating health centers

On October 14, 2026, U.S. Department of Health and Human Services (HHS) Secretary Robert F. Kennedy, Jr. and HRSA Administrator Tom Engels announced an initiative for HRSA-supported health centers to encourage healthy lifestyles and prevent chronic disease. Following a launch event at Piedmont Access to Health Services (PATHS) in Chatham, VA, HRSA invited HRSA-supported health centers to sign the Healthy Lifestyle Commitment.

The following health center has signed the Healthy Lifestyle Commitment. Additional participating health centers will be added soon.
