How to submit
When it’s time for you to submit:
- Your points of contact will receive an email from HRSA Electronic Handbooks (EHBs).
- Complete the BPHC Contact Form if your contacts do not receive an email to submit.
- Your Authorizing Official (AO) must submit through EHBs by your submission deadline. If you do not, you risk the termination of your LAL designation and associated benefits.
- Request technical assistance prior to the deadline, if needed.
What you’ll report
- Progress made since your last application submission (Renewal of Designation [RD] or AC)
- Expected progress for the rest of the current period of certification
- Projected changes for the upcoming period of certification

* Look-Alikes with a January 31, 2027, April 30, 2027, and May 31, 2027, period of designation end date will receive a one-year extension and will instead submit a Look-Alike Annual Certification.
