Baseline Number | 0.00 |
Baseline Data Source | Needs Assessment |
Current Number* | 4.00 |
Objective Target | 16.00 |
Percent Progress Toward Objective Target | 25.00% (Calculated based on the Current Number entered.) |
Objective Impact Narrative | Health centers will benefit from ongoing policy surveillance and written analysis on issues that may impact access to community health center service and/or insurance coverage. Health center staff will receive timely information related to state and federal policies through education opportunities used to improve access to services at the local level. Peer Learning Groups (PLGs) are the primary way our PCA provides training and technical assistance. In PLGs, groups will discuss promising practices, network with peers, and learn practical strategies to increase participation in health insurance coverage and engagement in primary health care. |
Objective Impact Narrative Progress* | During the reporting period (from July 1 - December 31), our organization initiated a three-year effort to provide weekly updates to relevant state and federal policies through mixed-mode education for health centers regarding increasing access to comprehensive, culturally competent, high-quality primary health care services, which included planning and implementation of nine (9) one-on-one calls to collect information regarding needs and one (1) group hybrid webinar that were recorded on topics identified during the surveillance calls. |

Training and Technical Assistance (T/TA) Sessions Target - Progress Number* | 10 |
Training and Technical Assistance (T/TA) Session Target - Progress Narrative* | For this objective, our organization facilitated nine (9) one-on-one calls and one (1) webinar. This reached over 40 individuals, ensuring our organization remains on target to meet the participation target. |
Participant Rating of Training and Technical Assistance (T/TA) Usefulness Target | 4.00 |
Participant Rating of Training and Technical Assistance (T/TA) Usefulness Target - Progress Number* | 4.50 |

Activities (minimum of 2 activities per objective)
Activity Name | Reporting Lab: Clinical Outcomes Metrics |
Activity Audience* | Community health centers in the state |
Activity Description |
Over 38% of respondents to the 2024 State T/TA Needs Assessment identified a need for T/TA on how to leverage data better to guide and inform clinical quality. We will develop and deliver quarterly virtual data quality PLGs with at least 25 participants from health centers. T/TA will be based on the needs identified and a review of best practices. The PLGs will be recorded and shared broadly with all health centers in the state through posting on our website and sharing through our newsletter. |
Person/Group Responsible | Chief Medical Information Officer, Clinical Data Scientist, Physician Informatics, Data Analyst, Specialist, Public Health Informatics. |
Target Start Date | 07/01/2024 |
Target End Date | 06/30/2025 |
Expected Activity Outcome |
After the PLGs, we will develop resources for health center staff (including data and IT personnel) to use while measuring and reporting clinical data. We anticipate at least 150 downloads and/or views of the resources developed. |
Activity Progress Update* | We promoted two virtual data-quality PLGs to all health centers, and 30 staff from 20 health centers participated. We also created and widely shared two new resources for health center staff. To date, the two new resources have received 200 downloads and/or views. |
Anticipated Activity Progress* | Between January 1, 2025, and June 30, 2025, we will host two virtual data quality PLGs and create three additional resources for health center staff to improve reporting workflows, data collection methodologies, and data standardization. |
