Overview

A SAC or SAC-AA application is a request for federal financial assistance to continue primary health care services in an area listed in the Service Area Announcement Table (SAAT). These areas are served by current Health Center Program award recipients whose periods of performance end in FY 2024; in instances of a SAC-AA, service areas may be competed off-cycle.

Who can apply

Eligible organizations include domestic public or nonprofit private entities, such as tribal, faith-based, or community-based organizations, that propose to serve an announced service area and its population(s) to ensure continued access to primary health care services. How to apply

SAC and SAC-AA applications require a two-phase submission process.

Phase 1 - Grants.gov: Must be completed and successfully submitted to Grants.gov by 11:59 p.m. ET on the date noted in the NOFO.

Phase 2 - HRSA EHBs: Must be completed and successfully submitted by 5:00 p.m. ET on the date noted in the NOFO.
