Overview
Fiscal year (FY) 2021 School-Based Service Sites (SBSS) funding is available for Health Center Program award recipients on a competitive basis. SBSS funding will expand access to health center services by increasing the number of patients who access comprehensive primary health care services through Health Center Program service delivery sites located at schools.
Health centers will accomplish this by:
- adding new Health Center Program service delivery sites at schools, or
- expanding comprehensive primary health care services at existing Health Center Program service delivery sites located at schools.
One-time funding will be available in year 1 for equipment purchases and minor alterations and renovations (A/R) (with or without equipment purchases).
Focus Areas
Your application must demonstrate how you will use SBSS funds in your current service area to increase the number of patients served at school-based service sites by adding or expanding one or more of the following services:
- General primary medical care,
- Mental health,
- Substance use disorder,
- Oral health,
- Vision, and/or
- Enabling services.
Eligible Applicants
Only Health Center Program award recipients (organization funded under Section 330(e), (g), (h), and/or (i) of the Public Health Service Act, as amended) with active H80 grant awards may apply for SBSS funding.
How to Apply
SBSS applications require a two-phase submission process.
Phase 1 - Grants.gov: Must be completed and successfully submitted to Grants.gov by 11:59 pm ET on March 22, 2021.
Phase 2 - HRSA EHBs: Must be completed and successfully submitted by 5:00 pm ET on April 20, 2021.

One-Time Funding
Of the $200,000 SBSS annual funding ceiling, applicants may request one-time funding up to $150,000 in year 1 for one-time costs for equipment and/or minor A/R. The following samples and templates are for planning purposes only.
