Table descriptions

The UDS clinical tables provide information on:

• Selected diagnoses and services (including medical, mental health, substance use disorder, vision, dental, patient support, and upstream drivers of health)
• Preventive care and screening
• Maternal and children’s health
• Disease management

Table Fact Sheets define key terms, highlight changes from the prior year, explain how UDS data are used, provide helpful hints for completing the UDS tables, and outline the importance of cross-table relationships and considerations.

Table 6A Code Changes (PDF - 428 KB) outlines changes to Current Procedural Terminology (CPT), International Classification of Diseases (ICD), and Value Set Object Identifier (OID) codes between the current reporting year and the prior year, by diagnosis or service line listed on UDS Table 6A.

UDS Clinical Quality Measures (CQM) Criteria (PDF - 335 KB) describes each of the CQMs reported in UDS Tables 6B and 7. This resource includes details about the denominator, numerator, any exceptions or exclusions to apply, as well as highlighting changes from prior reporting years and provides the major differences between CQMs captured in the UDS and CMS’ electronic clinical quality measures (eCQMs). 

eCQM Encounter Code Guide (XLSX - 230 KB) This downloadable workbook contains details from each electronic clinical quality measure's (eCQMs) specifications, outlining the qualifying encounter types that are included in the denominator of the CQMs on Tables 6B and 7.

2025 UDS Clinical Measures Exclusions and Exceptions (PDF - 373 KB) describes how to report the clinical measure numerators and denominators as the measure stewards intended.

UDS HIV Prevention and Linkage to Care Reporting (PDF - 286 KB) provides relevant instruction and CPT, ICD, and OID codes to help identify patients to be reported for the Table 6B HIV Linkage to Care Line 20 measure, Table 6B HIV Screening Line 20a, Table 6B HIV Tests Line 21, and the Table 6A Pre-Exposure Prophylaxis (PrEP)-associated prescribing and management Line 21e.
